Coverage is subject to AXA's Policy Terms and Conditons. Students are advised to contact the SUTD Office of Student Administration for the actual Terms and Conditions.

 

Please refer below for a summary of the Insurance Coverage:


GROUP PERSONAL ACCIDENT (GPA)

  • Covers students for bodily injury caused solely and directly by an accident and not by illness, disease or gradual physical or mental wear and tear.

  • "Accident" is defined as "a sudden, unexpected and external event that solely and independently results in bodily injury and which does not include any illness, disease or degenerative process”.

  • Territorial Limit – 24 hours worldwide.

GENERAL PRACTITIONER (GP)

  • Covers GP consultation, standard medicine and simple tests (eg. x-ray and lab tests).
  • Student may have to top-up for non-standard medicine (original/branded/patented medicine) if prescribed or pay a surcharge for visits after normal clinic operating hours.
  • Physiotherapy referred by a GP is not covered.

 

For Referrals to a Singapore Government Restructured Hospital for Specialist Treatment and/or Hospitalisation/Surgery:

  • If you are referred by a Panel GP Clinic, you are considered a Private patient;
  • To qualify as a subsidised patient, you must be a Singaporean or Singapore PR, referred by a Polyclinic or A&E of a Singapore Government Restructured Hospital and without reference to a Specialist by name.

Please note that the list of panel clinics may be updated periodically. Please check this website for the latest list before you visit the clinic. You may also wish to contact the clinic before visiting in case there is a change in operating hours.

Please bring your SUTD Student Card and your NRIC/FIN Card when you visit the panel clinic and show it to the clinic staff. Otherwise, you will have to pay first and submit a claim for reimbursement.

 

SPECIALIST (SP)

  • Covers visits to a Specialist in a Singapore Government Restructured Hospital.
  • Consultation, prescribed medicine, diagnostic tests and physiotherapy recommended by the Specialist are covered up to $1,000 per year.
  • The Specialist must be referred by a GP or A&E. Visits to the Specialist without referral is not covered.
  • Treatment by Private Specialists/Physiotherapists is not covered.
  • A new referral is required if the last visit to the Specialist was more than 1 year ago.
  • Please pay first and submit a claim for reimbursement.
 

OUTPATIENT MENTAL HEALTH

  • Covers visits to a Psychiatrist in a Singapore Government Restructured Hospital.
  • For Local (Singaporean/SPR) students, the coverage is included in the Specialist Limit of $1,000 per year. For International students, the coverage is up to $2,000 per year.
  • The Psychiatrist must be referred by a Physician or an SUTD Counsellor. Visits to the Psychiatrist without referral is not covered.
  • Treatment by Private Psychiatrists is not covered.
  • Please pay first and submit a claim for reimbursement.

HOSPITALISATION &/OR SURGERY (GHS)

For Local (Singaporean/SPR) students:

  • B2 ward (6 bedded)
  • With sub-limits for surgery, hospital services etc (please refer to the details here)
  • No annual limit

For International students:

  • B1 ward (4 bedded)
  • Limit of $50,000 per year

Letter of Guarantee (LOG)

  • A LOG is a facility provided by the insurer if you are hospitalised or have surgery in Singapore. With a LOG, the hospital will bill the insurer directly. Thus, you will not have to pay the cash deposit or the hospital charges on discharge, for the amount covered by the insurance.

Notes:

  • Territorial Limit – 24 hours worldwide for emergency treatment for accident or acute illness while overseas where immediate medical attention is required (does not cover non-emergency, elective and/or intentionally booked treatment overseas).
  • Medical expenses incurred in an overseas hospital will be limited to equivalent treatment in a B1/B2 ward at Singapore General Hospital (SGH).
  • If a student prefers to return home for treatment (hospitalisation/surgery), please inform us using the Contact Form to obtain approval in advance.

Letter of Guarantee (LOG)


A LOG is a document issued by the insurer for hospitalisation and/or surgery. With a LOG, the hospital will waive the cash deposit and payment of the hospital bill up to the limits of the insurance and subject to the policy terms and conditions. The hospital will bill the insurer directly.

Apply for LOG

 

  • The LOG is only applicable under the Group Hospitalisation & Surgical Insurance (GHS).
  • The LOG is not applicable for treatment outside Singapore.
  • For treatment overseas, please pay first and submit a claim for reimbursement.
  • Please note that LOG will not be issued for medical treatment that is not covered or if the required medical information cannot be obtained prior to hospital admission.
  • For emergencies, please contact MYCG immediately or settle the bill first and submit the claim later.

 

Step 1

Complete this form and submit to MYCG at least 5 working days before the scheduled hospital admission/surgery. For emergency admission, please contact MYCG as soon as possible.

Step 2

Upload these documents:

  1. Completed Release of Medical Information Form (ROMIF) →

  2. Copy of Referral Letter from GP to Specialist, if any

  3. Copy of *Financial Counselling/ Admission Form/ Estimated Bill/ Day Surgery Authorisation Form

*The hospital will give these documents to the patient when the admission date is confirmed/during the financial counselling session. The documents will contain the estimated bill, diagnosis and treatment information - this information is required to process the LOG.

Step 3

The LOG will be faxed to the hospital 1 to 2 days prior to admission after it is approved by the insurer. A copy will be emailed to the student.

 

Step 4

Upon discharge from the hospital, please submit:

  1. Discharge Summary by email to customercare@mycg.com.sg and
  2. Original Pre and Post hospitalisation/surgery bills by post to MYCG.

How to make a claim?

 

Step 1

Prepare/obtain the following documents:

Step 2a - Outpatient Bills (including pre and post hospitalisation/surgery bills)

  • Scan a clear copy of the documents to be submitted
  • Email the documents to customercare@mycg.com.sg
  • The subject header of the email should be

"NEW / NAME OF STUDENT / SUTD" for new claims or
"FOLLOW UP / NAME OF STUDENT / SUTD" for follow-up claims

  • You must keep the original bills for up to 1 year as the insurer may request for verification at anytime or for audit

 

Step 2b - Hospitalisation/Surgery Bills

  • If LOG has been issued, please email the Discharge Summary to customercare@mycg.com.sg
  • If LOG has not been issued, please send the original documents to MYCG by post.

 


Generally, claims will be processed within 30 days after receipt of complete documents and information.

The student will be notified of the result of the claim by email. For approved medical expense claims, the reimbursement will be credited into the student’s bank account.

  • The subject header of the email should be "FOLLOW UP / NAME OF STUDENT / SUTD"

CLAIM NOTIFICATION

Claims should be submitted within 30 days of treatment. If more time is required, please submit this form to notify MYCG.
