If you wish to purchase the insurance for your dependants, please submit the application form below and pay the premium by 7 Mar 2025. Enrolment will not be accepted after this date. The insurance will be effective from 1 Feb 2025.

Step 1

Please note that by submitting this application, you confirm that you have received a copy of (a) Your Guide to Health Insurance (b) Your Guide to Life Insurance and (c) Product Summary and have read and understood the contents of both documents.

Step 2
Premium is payable by bank transfer. MYCG will email you the bank details for the transfer after receiving your online application.

Step 3
Make payment and email the payment confirmation to lohmayyee@mycg.com.sg.

Step 4
You will receive an email from MYCG to confirm the purchase.
