If you wish to purchase the insurance for your dependants, please submit the application form below and pay the premium by 12 Feb 2026. Enrolment will not be accepted after this date. The insurance will be effective from 1 Feb 2026.

Step 1

Please note that by submitting this application, you confirm that you have received a copy of (a) Your Guide to Health Insurance and (b) Your Guide to Life Insurance and have read and understood the contents of both documents.

Step 2
Premium is payable by bank transfer. MYCG will email you the bank details for the transfer after receiving your online application.

Step 3
Make payment and email the payment confirmation to lohmayyee@mycg.com.sg.

Step 4
You will receive an email from MYCG/Income to confirm the purchase.
