The insurance covers reasonable expenses incurred for medically necessary outpatient treatment (as specified in the table of benefits), hospitalisation and/or surgery for an illness or injury in a Singapore Government Restructured Hospital subject to the policy limits, terms and conditions.

A Letter of Guarantee (LOG) is a document issued by the insurer for hospitalisation and/or surgery. With a LOG, the hospital will waive the cash deposit and payment of the hospital bill up to the limits of the insurance and subject to the policy terms and conditions. The hospital will bill the insurer directly.

The LOG is not applicable for outpatient treatment or treatment outside Singapore. For both, please pay first and submit a claim for reimbursement.
The issuance of a LOG does not constitute as an approval on any claim/claim amount in respect to the hospital admission under the insurance plan. Coverage will be confirmed after receipt of the final hospital bill and discharge summary, subject to the policy terms and conditions.
Email the following documents to groupclaim@income.com.sg and claims@mycg.com.sg at least 5 working days before the scheduled admission/surgery:
Care Cost Form / Financial Counselling Form / Admission Form / Day Surgery Authorisation Form (The hospital will give these documents to the patient when the admission date is confirmed. The documents should contain the diagnosis, surgical code (if any) and estimated bill.)
Referral letters, tests reports etc. if any
The insurer will assess the case.
If approved, the LOG will be emailed to you one day before the admission.
After discharge, please email the Discharge Summary and Final Hospital Bill to groupclaim@income.com.sg and claims@mycg.com.sg.
Pre and post hospitalisation/surgery bills should be submitted via email to groupclaim@income.com.sg and cc claims@mycg.com.sg. Please state your full name and polytechnic in the email subject header.

Note:
Treatment date must be within 6 months of the date of referral.
Original invoices and receipts must be kept for 6 months from the date of treatment and provided to the insurer on request.
Generally, medical expense claims will be processed within 30 days upon receipt of complete documents/information.
Notification of the result of the claim or request for documents/information will be sent to the student’s email address stated on the claim form.
Approved medical expense claims will be credited into the student’s bank account.

Treatment at a hospital 24-hr walk-in clinic may not covered (for example, the 24hr walk-in clinic at KKH is a specialist clinic - thus referral from a GP is required.) For emergencies, please seek treatment at the hospital A&E.
Treatment at a Government Restructured Outpatient Specialist Clinic operating out of Private Hospital/Clinic premises is not covered.
