Please refer to the NUS Student Insurance website for details of the insurance.

 

Insured Persons will enjoy similar coverage as Graduate Students except:

  1. Pre-existing Conditions are not covered unless the Insured Person has been insured continuously for twelve (12) months under this policy.
  2. The Insurance is applicable in Singapore only. There is no coverage in your home country or when you travel outside of Singapore.
  3. Under Group Outpatient Specialist (SP) Insurance, treatment at a Singapore Government Restructured Hospital A&E is covered up to $100 per visit, maximum 2 visits per year.

Important Note

  • The master policy is renewable annually. The next renewal is on 31/07/2018. As such, you can only purchase the insurance up to 31/07/2018. We will inform you in June 2018 on the renewal terms from 01/08/2018 onwards.
  • The period of insurance that you can purchase is the "official period" as advised by NUS plus maximum ten (10) additional days in total for the period immediately before or after the "official period".

Premium Payable

The premium is $24.00 per month, subject to minimum of $144.00 including 7% GST.

  • For period less than 6 months, the premium is $144.00 including 7% GST.
  • For period more than 6 months, the premium is $24.00/mth x no. of months including 7% GST.

The full premium must be paid. Monthly premium is not accepted.

 

Premium Calculation - Number of Months

  • Example: If the period of insurance is from 15/09/2017 to 31/07/2018, the premium payable will be for 11 months.
  • Example 2: If the period of insurance is from 01/02/2018 to 31/07/2018, the premium payable will be for 6 months (minimum premium).

By ATM/Bank Transfer to AXA’s OCBC Bank Account.

The period of insurance that you can purchase is the "official period" as advised by NUS plus maximum ten (10) additional days in total for the period immediately before or after the "official period".

 

  • If you do not fall within the abovementioned group, please do not apply but contact us.
  • Coverage at point of claim is subject to confirmation of your status as mentioned above, subject to (i) verification from NUS that you eligible for the insurance and (ii) confirmation from NUS on the "official period" that you are eligible for coverage. The coverage will terminate once you no longer meet the eligibility requirements. The premium must have been paid.
  • We will only check on your eligibility in event of claim. Please note that claims will not be payable if you are not eligible/approved even if you have paid for the insurance.

New Applications

 

Step 1

Please arrange payment of the insurance by ATM/bank transfer and email a screenshot of the transaction confirmation to MYCG.

 

Step 2

Complete this New Application Form (including payment details if you choose to pay by ATM/bank transfer).

 

Step 3

The Insurance Certificate will be emailed to you within 2 working days. If you do not receive the certificate, please contact MYCG to enquire.

LOG


A LOG is a document issued by the insurer for hospitalisation and/or surgery. With a LOG, the hospital will waive the cash deposit and payment of the hospital bill up to the limits of the insurance and subject to the policy terms and conditions. The hospital will bill the insurer directly.

Apply for LOG

 

Step 1

Complete this form and submit to MYCG at least 5 working days before the scheduled hospital admission/surgery. For emergency admission, please contact MYCG as soon as possible.

Step 2

Upload these documents:

  1. Completed Release of Medical Information Form (ROMIF) →

  2. Copy of Referral Letter from GP to Specialist, if any

  3. Copy of *Financial Counselling/ Admission Form/ Estimated Bill/ Day Surgery Authorisation Form

*The hospital will give these documents to the patient when the admission date is confirmed/during the financial counselling session. The documents will contain the estimated bill, diagnosis and treatment information - this information is required to process the LOG.

Step 3

The LOG will be faxed to the hospital 1 day prior to admission after it is approved by the insurer. A copy will be emailed to the student.

 

Step 4

Upon discharge from the hospital, please email the following to MYCG:

  1. Discharge Summary and
  2. Pre and Post hospitalisation/surgery bills.

How to make a claim?

 

Step 1

Prepare/obtain the following documents:

      GPA - Group Personal Accident Insurance
      GHS - Group Hospitalisation & Surgical Insurance
      H&S - Hospitalisation & Surgery
      LOG - Letter of Guarantee

Step 2

  • Please email the claim documents to MYCG. Please keep the original documents for up to one (1) year in case the insurer may require for verification or audit.
  • If you are submitting a claim under the Group Hospitaisation & Surgical (GHS) Insurance and did not apply for LOG, please submit the original Final Hospital Bill by post or by hand to MYCG.

Follow-Up Claims

Please email the Follow-up Claim Form and Bills to MYCG. Please keep the original documents for up to one (1) year in case the insurer may require for verification or audit.


Generally, claims will be processed within 30 days after receipt of complete documents and information.

The student will be notified of the result of the claim by email. For approved medical expense claims, the reimbursement will be credited into the student’s bank account.
