How to make a claim?

 

Step 1

Prepare/obtain the following documents:

      GPA - Group Personal Accident Insurance
      GHS - Group Hospitalisation & Surgical Insurance
      H&S - Hospitalisation & Surgery
      LOG - Letter of Guarantee

Accident Claim Form - For Local Undergraduates
Combined Claim Form - For Other Students

 

Step 2

Please keep a copy for your records.

 

Step 3

Send the documents by post to MYCG or hand it to our representative who will be at UHC every Wednesday and Thursday between 10 am to 12 noon.


Generally, claims will be processed within 30 days after receipt of complete documents and information.

The student will be notified of the result of the claim by email. For approved medical expense claims, the reimbursement will be credited into the student’s bank account.


Follow-up Claims

  1. Please complete the Follow-up Claim Form.
  2. Enclose the original bills/documents to be submitted.
  3. Send the documents by post to MYCG or hand it to our representative who will be at NUS UHC every Wednesday and Thursday between 10 am to 12 noon.

Claim Notification

Claims should be submitted within 30 days of treatment. If more time is required, please submit this form to notify MYCG.

Claims which are submitted late may not be processed. Please submit early.
