How to make a claim?

 

Step 1

Prepare/obtain the following documents:

GPA - Group Personal Accident Insurance
GHS - Group Hospitalisation & Surgical Insurance
H&S - Hospitalisation & Surgery
LOG - Letter of Guarantee

Step 2

  • Please email the claim documents to MYCG. Please keep the original documents for up to one (1) year in case the insurer may require for verification or audit.

  • If you are submitting a claim under the Group Hospitaisation & Surgical (GHS) Insurance and did not apply for LOG, please submit the original Final Hospital Bill by post to MYCG.


Generally, claims will be processed within 30 days after receipt of complete documents and information.

The student will be notified of the result of the claim by email. For approved medical expense claims, the reimbursement will be credited into the student’s bank account.


FOLLOW-UP CLAIMS

Please email the Follow-up Claim Form and Bills to MYCG. Please keep the original documents for up to one (1) year in case the insurer may require for verification or audit.


CLAIM NOTIFICATION

Claims should be submitted within 30 days of treatment. If more time is required, please submit this form to notify MYCG.

Claims which are submitted late may not be processed. Please submit early.
