  • According to the Personal Data Protection Commission (PDPC), from 1 September 2019, organisations are not allowed to collect NRIC or other national identification numbers unless required by law or necessary for verification of an individual's identity to high fidelity. As such, there are some changes to log in to the Cliniflex app and portal.

  • Username
    Before 1 Sep 2019 : Username = NRIC/FIN No.
    From 1 Sep 2019 : Username = SUTD email in this format “1234567@mymail.sutd.edu.sg” where 1234567 is your SUTD ID No.

  • Thus, all students will have to re-register on the app/portal to be able to log in

  • f you are unable to register or log in, please take a screenshot of the error messages and email to customercare@mycg.com.sg or call 6904 4546.


Important:

  • The SUTD Student Card will no longer be recognised at the panel clinics;

  • Payments made at the panel clinics will not be reimbursible - you must show your e-card to the clinic staff.

  • you may wish to contact the clinic to confirm their operating hours before your visit

The student insurance accidental bodily injury and illness including mental illness.

Coverage is subject to Income's Policy Terms and Conditions. Students are advised to contact the SUTD Office of Student Administration for the actual Terms and Conditions.


GROUP PERSONAL ACCIDENT (GPA)

  • Covers students for bodily injury caused solely and directly by an accident and not by illness, disease or gradual physical or mental wear and tear.

  • "Accident" is defined as "a sudden, unforeseen and fortuitous event which shall independently of any other cause be the sole and direct cause of bodily injury.”.

  • Territorial Limit – 24 hours worldwide.

  • For medical expenses, please pay first and submit a claim for reimbursement.


GENERAL PRACTITIONER (GP)

  • Covers GP consultation, prescribed medicine and simple tests (eg. x-ray and lab tests).

  • Show the E-Card to the clinic staff and you will not have to pay for consultation, prescribed medication and simple lab tests for covered medical conditions. (Please note that if you did not show the e-card and had to pay at the panel clinic, the claim may be processed as non-panel clinic claim.)

  • View the list of panel clinics from the app or portal.

Note:

For Referrals to a Singapore Government Restructured Hospital for Specialist Treatment and/or Hospitalisation/Surgery:

  • If you are referred by a Panel GP Clinic, you are considered a Private patient;

  • To qualify as a subsidised patient, you must be a Singaporean or Singapore PR, referred by a Polyclinic or A&E of a Singapore Government Restructured Hospital and without reference to a Specialist by name.

SPECIALIST (SP)

  • Covers visits to a Specialist in a Singapore Government Restructured Hospital.

  • Covers consultation, prescribed medicine and diagnostic tests recommended by the Specialist.

  • Annual Limit of $1,000.

  • The Specialist must be referred by a GP or A&E. Visit to the Specialist without referral is not covered.

  • Treatment by Private Specialist is not covered.

  • Covers treatment in Singapore only, except for international students who return to their home country for treatment. Charges incurred overseas shall be reimbursed up to overseas charges or B1 charges in a Singapore Government Restructured Hospital for similar treatment, whichever is lower.

  • A new referral is required if the last visit to the Specialist was more than 6 months ago.

  • Please pay first and submit a claim for reimbursement.

Outpatient Mental Health

  • Covers visits to a Psychiatrist, Psychologist or Neurologist in a Singapore Government Restructured Hospital.

  • For Local (Singaporean/SPR) students, the coverage is included under the Specialist Limit of $1,000 per year. For International students, the coverage is up to $2,000 per policy year.

  • Referral by a Physician or an SUTD Counsellor. Visits to the Psychiatrist without referral is not covered.

  • Treatment by Private Psychiatrist is not covered.

  • Covers treatment in Singapore only, except for international students who return to their home country for treatment. Charges incurred overseas shall be reimbursed up to overseas charges or B1 charges in a Singapore Government Restructured Hospital for similar treatment, whichever is lower.

  • Please pay first and submit a claim for reimbursement.


HOSPITALISATION &/OR SURGERY (GHS)

  • Covers hospitalisation and surgery expenses incurred in a Singapore Government Restructured Hospital. Treatment in a Private Hospital is not covered.

  • Territorial Limit – 24 hours worldwide for emergency treatment for accident or acute illness while overseas where immediate medical attention is required (does not cover non-emergency, elective and/or intentionally booked treatment overseas).

  • Extends to cover international students who return to their home country for treatment.

  • Charges incurred overseas shall be reimbursed up to overseas charges or B2/B1 charges and plan limits in a Singapore Government Restructured Hospital for similar treatment, whichever is lower.

  • Extends to cover expenses incurred for stay in a Community/RehabilitationHospital registered and approved by the Ministry of Health of Singapore if recommended by the attending Registered Medical Practitioner up to
    maximum 31 days and $5,000, subject to the benefit limits of the insurance.

  • You can apply for Letter of Guarantee (LOG) to cover the hospital expenses.

For Local (Singaporean/SPR) students:

  • B2 ward (6 bedded)

  • With sub-limits for surgery, hospital services etc

  • No annual limit

For International students:

  • B1 ward (4 bedded)

  • Limit of $50,000 per year

A LOG is a document issued by the insurer for hospitalisation and/or surgery. With a LOG, the hospital will waive the cash deposit and payment of the hospital bill up to the limits of the insurance and subject to the policy terms and conditions. The hospital will bill the insurer directly.

  • The LOG is only applicable under the Group Hospitalisation & Surgical Insurance (GHS).

  • The LOG is not applicable for treatment outside Singapore.

  • For treatment overseas, please pay first and submit a claim for reimbursement.

  • Please note that LOG will not be issued for medical treatment that is not covered or if the required medical information cannot be obtained prior to hospital admission.

  • For emergencies, please contact MYCG immediately or settle the bill first and submit the claim later.

 

Step 1

Complete this form and submit to Cliniflex & MYCG at least 5 working days before the scheduled hospital admission/surgery. For emergency admission, please contact MYCG as soon as possible.

Step 2

Email these documents to cliniflex@adepthealth.com.sg and copy care@mycg.com.sg:

  1. Completed Authorisation Form →

  2. Copy of Referral Letter from GP to Specialist, if any

  3. Copy of Test Reports, if any

  4. Copy of *Financial Counselling/ Admission/ Care Cost/ Day Surgery Authorisation Form

*The hospital will give these documents to the patient when the admission date is confirmed/during the financial counselling session. The documents will contain the estimated bill, diagnosis and treatment information - this information is required to process the LOG.

Step 3

The LOG will be emailed to the hospital 1 to 2 days prior to admission after it is approved by the insurer. A copy will be emailed to the student.

 

Step 4

Upon discharge from the hospital, please submit:

  1. Discharge Summary by email to cliniflex@adepthealth.com.sg and copy care@mycg.com.sg and

  2. Submit Pre and Post hospitalisation/surgery bills via the Cliniflex by Income app to claim.
